Psychiatric consultation was requested by a thoracic surgeon for a 21-year-old Indian man from the Cuna tribe that inhabits the San Blas Islands on the Atlantic side of the Isthmus of Panama. The man had attempted to kill himself by shooting a bullet into his chest. When he was seen, he was already recovering from his wounds and was about to be discharged from the surgical ward.
